If you are interested in using a sperm donor for either artificial insemination or in vitro fertilization, you must
complete these simple steps:

| I. | Physician Consultation: |
Arrange for a consultation with your CFA physician who will request all appropriate medical screening, including bloodwork (Blood Type,
HIV, Hepatitis B screening) and anatomical studies. The possible use of fertility medications will also be reviewed at that time.

| II. | Choose donor: | Select your donor choices
from the available donor list.
|
| III. | Contact CFA Sperm Donor Coordinator: |
All patients planning to pursue sperm donation must first meet with CFA's Sperm Lab Manager, Lynn Kurjiaka, to schedule the required
screening test results; and to choose the ideal sperm donor.

| IV. | Complete paperwork: | Review and complete
all relevant consent forms regarding sperm donation.
